Providing Support to General Practices to Promote Patient-Centered Care Options

Hepatitis Queensland is a community based, non-government organisation representing the interests of people affected by, or at risk of viral hepatitis. They want all Queenslanders to enjoy the benefits of a healthy liver and ensure everyone has access to appropriate medical treatment, up to date and accurate health information and opportunities to learn and understand how to manage their own wellbeing.

Their mission is to address stigma, improve the lives of those affected by viral hepatitis and meet emerging liver health challenges.

The Be Liver Smart project provides support to general practices to promote patient-centered care options for the management of hepatitis C in the community. Participant practices benefit from expert clinical support including fast tracked referrals, training and education and additional practice support through our Be Liver Smart community nurse. Be Liver Smart has already proven to be a great health initiative for the community with more than 61 general practices engaged in our services since the inception of this project. To date, we are proactively supporting 10 medical practices by providing monthly liver health clinics. Participation in this project generates a wide range of opportunities for practices and their patients.
